What services does CHANCE offer under the NDIS?
We are focused on providing Advanced Clinical Nursing and High Intensity Support including mental health recovery and management
We also support & build people's capacity to live independently in the community, supports to improve living skills, money and household management, social and communication skills, continence assessments and mental health recovery & behavioural management child and adult, under the scope of a Clinical Nurse/ Registered Nurse and Aboriginal Health Worker
Support with personal care, such as assistance with showering and dressing
Help around the home where the participant is unable to undertake these tasks due to their disability, such as assistance with cleaning and laundry, and even a transport service and accompanying participants to social, fun and recreational events
Our team are COVID 19 trained and compliant

Assistance with Daily Life
Assistance with Daily Life (ADL) is a support category for assistance or supervision of personal tasks during day-to-day life that enable the participant to live as independently as possible. These supports are provided individually to participants and can be provided in a range of environments, including their own home. We provide personal care, cooking, cleaning and gardening.
Community Nursing
Preventing illness and promoting health, our Clinical Nurse Specialist and Registered Nurses specialise in community health and complex care.
We take initiative in emergencies, are highly trained / skilled and have strong communication skills.
From complex wounds, to injections, catheter care, tracheostomy care, continence assessments, and just about anything nurse related- we have your back!


Don't forget about fun and recreation
Our support workers and nurses will be able to accompany you to your leisure / sport activities any day of the week. We also have team members that can accompany you to sporting and social events that require travel and long stay, Yay!
Transport service
Our team are ready to provide transport services, to and from your appointments, meetings, and leisure activities. Just contact us to book a time


Telehealth nursing
COVID 19 has really pushed the health system to go digital.Telehealth services are funded by the NDIS for participants to be able to phone their nurses / health care workers to discuss any pyshical/ mental health related matters, or you just need to talk to someone. You are able to use phone or video call for support. We are always happy and available to take your calls. We're all in this together :)
